  • SOLE PROPRIETOR
    • SSN of owner or TIN of business
    • Copy of filed Assumed Name Certificate (Trade Name Affidavit) or Business License


  • PARTNERSHIP or LIMITED PARTNERSHIP
    • TIN of partnership
    • Partnership agreement (if applicable) Deposit and Borrowing Authorizations or Business Depository Resolution signed by all partners
    • Copy of Filed Assumed Name Certificate if partnership conducts business under an assumed name


  • CORPORATION OR PROFESSIONAL CORPORATION
    • TIN of corporation (Incorporated companies or Professional Corporation only)
    • Copy of filed Articles of Incorporation
    • Certificate of Incorporation
    • Copy of filed Assumed Name Certificate if corporation conducts business under an assumed name


  • LIMITED LIABILITY COMPANY/PARTNERSHIP (LLC), (LPA), (LLP), (LLLP)
    • Certificate of Authority must be obtained from the State of Colorado if the LLC is registered in a different state.
    • TIN of LLC
    • Copy of filed Articles of Organization
    • Copy of name Registration (Optional)


  • UNINCORPORATED BUSINESS ASSOCIATION/ORGANIZATION
    • TIN of Association/Organization
    • Meeting minutes stating the individuals authorized to establish or conduct business on behalf of the club or organization, signed by the president or officers of the organization.
Corporations established in another state may require additional documentation and verification of their existence including verification that the entity can conduct business in the State of Colorado.
